Hinged Elbow External Fixator for Complex Elbow Trauma

The elbow external fixator is a specialized orthopaedic device designed to stabilise severely injured elbow joints while permitting controlled joint motion. Most clinical versions adopt a hinged structure aligned with the elbow’s natural rotation axis, connecting pins inserted into the humerus and forearm bones with an external frame system. It is widely selected for complicated elbow fracture-dislocations, open elbow injuries, unstable joints after internal fixation, and post-traumatic elbow stiffness.

Traditional long-term plaster cast immobilisation often leads to joint fibrosis, stiffness and permanent loss of movement range. In contrast, the elbow external fixator holds the joint in reduced position and counteracts disruptive forces on repaired ligaments and bone structures. Surgeons can adjust the hinge to limit unsafe movement and guide safe flexion and extension, enabling early rehabilitation. This balance between stability and mobility greatly lowers the risk of stiff elbow, a common disabling complication after severe elbow trauma.

It also brings clear benefits for cases with poor soft tissue conditions. Since the fixation frame sits outside the skin, surgeons can inspect wound swelling, skin integrity and neurovascular status conveniently without disturbing fracture stabilisation. It can serve as temporary bridging fixation before secondary definitive surgery, or as a lasting construct after ligament reconstruction and arthrolysis.

Proper surgical placement and postoperative monitoring are essential. Pin tract care, regular imaging follow-up and structured physiotherapy are required to reduce infection risk and maximise functional recovery. As a reliable solution for difficult elbow injuries, the elbow external fixator remains an indispensable tool in modern orthopaedic trauma care.
